FILLING OUT THE FORM: Circle the Package you would like to order. Mark an X by any options you would like. There is NO sales tax to be added to your order. The middle section only needs completed if you are ordering OPTION 3. The lower section please print your child’s first and last name as you want it to appear on the pictures (sportsmate). The school name is MYSA. If your child has switched schools since registration please notify MYSA. For the space marked Sport, please enter your child’s division (U6, U8G, U8B, etc.)

Please be prepared to pay by check or credit card. If you must pay with cash please bring the exact amount, we will not be able to make change. We must have an order form, even if you pay online. The form helps to identify your child.

THE SCHEDULE: The Schedule is for TEAM pictures. We like to have as many of the team members in the picture as possible so please arrive early. Individual photo’s can be taken at anytime. It is easiest for the photographer and MYSA if you get your photo taken immediately before or after your team photo.

It is recommended that U6 players NOT practice in their Jersey’s on photo day but instead slip the jersey on over the practice clothes while in line for pictures. Wipes will be available to clear any dirt off faces and hands but we will not have extra jerseys available.

WHO NEEDS AN INDIVIDUAL PICTURE? If you are not ordering photo’s or are only ordering Package G with no options, you do not need to have an individual picture taken but we would like you to be in the group photo if possible. For any other package and for the options, you will need an individual photo.

WHAT IF YOU CAN’T make it to the group photo? If you can’t be there for you group photo, you can still order pictures. You will not be in the team picture of the sportsmate but you will have the group photo along with your individual photo.
